Overview

We are seeking a proactive and business-minded IT Project Manager to lead and support a portfolio of strategic healthcare technology initiatives. This role will focus on advancing home health solutions, expanding new lines of business, and driving digital transformation through AI, automation, and system integrations.


This individual will play a critical role in aligning technical execution with business value, working closely with cross-functional stakeholders to deliver impactful, scalable solutions across the organization.


Key Responsibilities


Project & Program Delivery

  • Lead multiple concurrent IT projects and programs of moderate to high complexity, ensuring delivery on time, within scope, and aligned to business objectives
  • Build and manage detailed project plans, timelines, and resource allocations
  • Oversee interdependencies across interconnected initiatives and ensure seamless integration across systems
  • Proactively identify risks, dependencies, and blockers, and drive mitigation strategies

Strategic Initiatives You’ll Support

  • Expansion into new care delivery models:
    • Inpatient Rehabilitation Facilities (IRF)
    • Long-Term Acute Care Hospitals (LTACH)
  • Development of new capabilities to increase patient referrals and improve care coordination
  • Implementation of AI-driven automation for home health documentation
  • Enhancement of outcome-driven indicators and reporting to support better clinical and operational decisions
  • Delivery of integrations and updates to existing healthcare systems

Stakeholder & Business Alignment

  • Partner closely with business leaders, IT teams, and clinical stakeholders to understand goals and translate them into actionable project plans
  • Build strong relationships across leadership and functional teams to drive alignment and adoption
  • Clearly communicate project status, risks, and outcomes in a concise, direct manner

Financial & Resource Management

  • Track and manage project financials, including budgets, forecasts, and resource allocation
  • Utilize tools such as ServiceNow PPM (or similar) to manage capacity and resourcing
  • Ensure projects deliver measurable business value and ROI

Technical & Delivery Execution

  • Collaborate with engineering and IT teams on system updates, integrations, and implementations
  • Apply knowledge of SDLC and ITIL frameworks to ensure structured and efficient delivery
  • Work across both Agile and Waterfall environments (no requirement to lead Scrum teams)


Qualifications


Required

  • Bachelor’s degree in Business, Information Technology, or a related field
  • Experience managing IT projects, ideally within healthcare or health solutions environments
  • Demonstrated ability to manage technical implementations, system integrations, and product delivery
  • Strong financial acumen with experience tracking project budgets and resources
  • Experience working with tools such as ServiceNow PPM, Jira, or similar platforms
  • Ability to work effectively with technical teams and translate business needs into technical execution
  • Strong communication skills with a concise, direct style

Preferred

  • Experience with healthcare systems
  • Exposure to AI, automation, or digital transformation initiatives
  • SAFe Agilist (5.0) certification or familiarity with Agile methodologies
  • Experience working in hybrid Agile/Waterfall environments
