This role will focus on advancing home health solutions, expanding new lines of business, and driving digital transformation through AI, automation, and system integrations.<\/p>\n<p><\/p>\n<p>This individual will play a critical role in aligning technical execution with business value, working closely with cross-functional stakeholders to deliver impactful, scalable solutions across the organization.<\/p>\n<p><\/p>\n<p><strong><u>Key Responsibilities<\/u><\/strong><\/p>\n<p><\/p>\n<p><strong>Project &#038; Program Delivery<\/strong><\/p>\n<ul>\n<li>Lead multiple concurrent IT projects and programs of moderate to high complexity, ensuring delivery on time, within scope, and aligned to business objectives<\/li>\n<li>Build and manage detailed project plans, timelines, and resource allocations<\/li>\n<li>Oversee interdependencies across interconnected initiatives and ensure seamless integration across systems<\/li>\n<li>Proactively identify risks, dependencies, and blockers, and drive mitigation strategies<\/li>\n<\/ul>\n<p><strong>Strategic Initiatives You\u2019ll Support<\/strong><\/p>\n<ul>\n<li>Expansion into new care delivery models:\n<ul>\n<li>Inpatient Rehabilitation Facilities (IRF)<\/li>\n<li>Long-Term Acute Care Hospitals (LTACH)<\/li>\n<\/ul>\n<\/li>\n<li>Development of new capabilities to increase patient referrals and improve care coordination<\/li>\n<li>Implementation of AI-driven automation for home health documentation<\/li>\n<li>Enhancement of outcome-driven indicators and reporting to support better clinical and operational decisions<\/li>\n<li>Delivery of integrations and updates to existing healthcare systems<\/li>\n<\/ul>\n<p><strong>Stakeholder &#038; Business Alignment<\/strong><\/p>\n<ul>\n<li>Partner closely with business leaders, IT teams, and clinical stakeholders to understand goals and translate them into actionable project plans<\/li>\n<li>Build strong relationships across leadership and functional teams to drive alignment and adoption<\/li>\n<li>Clearly communicate project status, risks, and outcomes in a concise, direct manner<\/li>\n<\/ul>\n<p><strong>Financial &#038; Resource Management<\/strong><\/p>\n<ul>\n<li>Track and manage project financials, including budgets, forecasts, and resource allocation<\/li>\n<li>Utilize tools such as ServiceNow PPM (or similar) to manage capacity and resourcing<\/li>\n<li>Ensure projects deliver measurable business value and ROI<\/li>\n<\/ul>\n<p><strong>Technical &#038; Delivery Execution<\/strong><\/p>\n<ul>\n<li>Collaborate with engineering and IT teams on system updates, integrations, and implementations<\/li>\n<li>Apply knowledge of SDLC and ITIL frameworks to ensure structured and efficient delivery<\/li>\n<li>Work across both Agile and Waterfall environments (no requirement to lead Scrum teams)<\/li>\n<\/ul>\n<p><\/p>\n<p><strong><u>Qualifications<\/u><\/strong><\/p>\n<p><\/p>\n<p><strong>Required<\/strong><\/p>\n<ul>\n<li>Bachelor\u2019s degree in Business, Information Technology, or a related field<\/li>\n<li>Experience managing IT projects, ideally within healthcare or health solutions environments<\/li>\n<li>Demonstrated ability to manage technical implementations, system integrations, and product delivery<\/li>\n<li>Strong financial acumen with experience tracking project budgets and resources<\/li>\n<li>Experience working with tools such as ServiceNow PPM, Jira, or similar platforms<\/li>\n<li>Ability to work effectively with technical teams and translate business needs into technical execution<\/li>\n<li>Strong communication skills with a concise, direct style<\/li>\n<\/ul>\n<p><strong>Preferred<\/strong><\/p>\n<ul>\n<li>Experience with healthcare systems<\/li>\n<li>Exposure to AI, automation, or digital transformation initiatives<\/li>\n<li>SAFe Agilist (5.0) certification or familiarity with Agile methodologies<\/li>\n<li>Experience working in hybrid Agile\/Waterfall environments<\/li>\n<\/ul>\n<p><\/p>\n<p><strong>What We\u2019re Looking
