Job Title: RN – Kidney Care Case Manager
Location: Indianapolis, IN (Hybrid – 3 to 5 days onsite per week)
Start Date: September 8th (flexible)
Type: 6-Month Contract-to-Hire (Potential 1-Year Contract)
License Required: Indiana or Compact RN License
Compensation: Aligned with existing IU Health Case Manager salary band
Overview:
IU Health is launching a new Kidney Care Case Management Program and is seeking 2 Registered Nurses to serve as the first clinical hires within this initiative. These nurses will help shape the program and support its integration into five nephrology outpatient office locations. Initial focus will be on North (11700 N Meridian St, Carmel, IN 46032) and IU West (1111 Ronald Reagan Pkwy, Avon, IN 46123), with potential rotation through other sites in the Indianapolis area
Key Responsibilities:
- Serve as the primary clinical point of contact for assigned nephrology offices.
- Collaborate with a team of 45 nephrologists and 30 MDs/NPs to coordinate care.
- Provide guidance during the early stages of the program’s rollout, helping define workflows and best practices.
- Actively support kidney care patients, even if not from a traditional case management background.
- Work closely with internal stakeholders and potentially support future integration of Care Navigators and Dietitians.
Requirements:
- Active RN license (IN or Compact)
- Kidney care/renal experience required (does not need prior case management background)
- Local to Indianapolis (reliable form of transportation)
- Excellent communication and adaptability skills
- Comfort working in a developing program with evolving structure
- Must NOT have recent experience at DaVita
Additional Details:
- Training will be fully remote through the end of the year
- Plan to embed into physical offices in early 2026
- Dress code: Business casual minimum
